Stanley Blvd-Lifestyle Rx, Livermore, CA 94550, effective June 8, 2026. The last patient day at this location is anticipated to be June 5, 2026. All Outpatient Services-Cardiac Rehab, Physical Medicine, and Speech Therapy will be relocated to 1015 E. Stanley Blvd., Livermore, CA 94550, ensuring continued access to care in the local community. The nearest Outpatient Physical and Sports Medicine services as of March 1, 2026 are: 1.
