Notice of Closure of Services Stanford Health Care Tri-Valley Pre-Op Testing Pursuant to California Health and Safety Code Section 1255.25, Stanford Health Care Tri-Valley is providing notice of its proposal to cease providing Outpatient Services Pre-Op Testing located at 5565 W. Las Positas Blvd., Suite 110B, Pleasanton, CA 94588 effective July 13, 2026, with last patient day being July 10, 2026. As of July 13, 2026, there will be one location that can provide similar outpatient preoperative testing services for patients receiving surgery at a Stanford Health Care Tri-Valley hospital-licensed location. It will be located at: Stanford Health Care-Anesthesia Preoperative Evaluation Clinic at Tri-Valley, 1133 E. Stanley Blvd., Suite 217, Livermore, CA 94550.
