REQUEST FOR PROPOSAL CITY OF DAYTONA BEACH SHORES, FLORIDA Employee Benefits Broker of Record NOTICE IS HEREBY GIVEN that the City of Daytona Beach Shores, Volusia County, Florida (City), is seeking proposals from qualified firms to provide employee benefits broker of record services for all employee health, dental, vision, life, disability, and other related employee benefit programs. Submissions will be received until 1:00 p.m. on Tuesday, May 26, 2026, at which time the receiving period will close. Any submission received after the time and date specified above will be returned unopened. Specifications are available on Demandstar and on file and available for inspection in the office of the City Clerk, who can be contacted at (386) 763-5364. Copies thereof may be obtained upon request to the City Clerk. All submissions are made online through Demandstar. The City reserves the right to reject any or all submittals and to waive any or all non-substantial irregularities in items received whenever such rejection is in the best interest of the City, or to accept the proposal which best serves the interest of the City. In accordance with Section 287.133(2)(a), Florida Statutes, any person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services to a public entity, may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity, and may not transact business with any public entity in excess of the threshold amount provided in Section 287.017), Florida Statutes, for Category Two for a period of thirty six (36) months from the date of being placed on the convicted vendor list.
