Closure of optometry practice
Dr. Jay Crump OD will be closing his practice, Florida Eye Care Clinic at 5306 Cortez Rd W STE 3, Bradenton FL 34210 effective March 6th, 2026. Patients may obtain a copy of their medical records until that date at the current office address. After that date, Dr. Crump will maintain custody and store records off-site and may be obtained by request at (941)321-1333. Thank you for allowing me to provide your eye care needs.
