DOB: 12/30/2016, MINOR CHILDREN NOTICE OF ACTION TERMINATION OF PARENTAL RIGHTS TO: Addie McClamma, Mother Address unknown YOU ARE HEREBY NOTIFIED that the State of Florida, Department of Children and Families, has filed a Petition to terminate your parental rights and permanently commit the following children for adoption: J.M. born on 12/9/2012 and C.B. born 12/30/2016. You are hereby commanded to appear on May 11, 2026, at 1:00 PM before the Honorable W. Collins Cooper at the Duval Courthouse, 501 W Adams Street, Courtroom 202, Jacksonville, FL 32202, for an ADVISORY HEARING.
