TO: FRANCISCO JAVIER MADERA FLORES, 609 Calle San Antonio, San Juan, Puerto Rico, 000915 YOU ARE NOTIFIED that an action for Petition for Dissolution of Marriage with Minor Children has been filed against you and that you are required to serve a copy of your written defenses, if any, to it on SHAUN J.
