DOB: 8/24/2012, MINOR CHILDREN / NOTICE OF ACTION TERMINATION OF PARENTAL RIGHTS TO: Christopher Smith, address unknown YOU ARE HEREBY NOTIFIED that the State of Florida, Department of Children and Families, has filed a Petition to terminate your parental rights and permanently commit the following children for adoption: S.S.S. born on 11/22/2008 and M.S.S. born on 08/24/2012.
