YOU ARE NOTIFIED that a civil action has been filed against you, as Beneficiary of the Glen Otto Douglas Trust (the Trust) and Donald Warren Douglas, as Trustee of the Trust, by Brian G Douglas, Beneficiary of the Trust and Plaintiff and by Timothy R Douglas as Beneficiary of the Trust and Plaintiff, in the Circuit Court of the Tenth Judicial Circuit in and for Hardee County, Florida. The nature of this action is to COMPEL the Trustee of the Trust to the binding arbitration of any and all disputes among and between beneficiaries of the Trust, the Trustee or any combination thereof, as required pursuant to the Trust’s terms.

An Affidavit of Diligent Search and Inquiry has been filed in this action stating that, after due diligence, personal service on you could not be effected because your residence is unknown or you are otherwise avoiding service.

You are required to serve a copy of your written defenses, if any, to the Complaint/Petition on Plaintiff’s attorney, whose name and address is:
