DOB: 12/28/2018 MINOR CHILD TO: TAJLYN PREVILONG, ADDRESS UNKNOWN YOU ARE HEREBY NOTIFIED that the State of Florida, Department of Children and Families, has filed a Petition to terminate your parental rights and permanently commit the following child for adoption: Y.M. born on 12/28/2018.
