NOTICE OF AMBULATORY SURGERY CENTER CLOSURE The FHCP ASC Orange City, located at 2777 Enterprise Road, Orange City, FL 32763, will be closed effective April 27, 2026. Patient medical records will be retained in a confidential and secure manner and will be available to patients or their authorized representatives.
