NOTICE IS HEREBY GIVEN, pursuant to  456.057, Fla. Stat. (2014) and Rule 64B8-10.002, F.A.C., that Daniel L. Spitz, MD, FACP is retiring from the practice of medicine with Florida Cancer Specialists, P.L., d/b/a Florida Cancer Specialists &amp; Research Institute, effective as of March 30, 2026 and that Florida Cancer Specialists &amp; Research Institute has been designated as records owner; thus, any requests by patients for copies of medical records can be made directly to Florida Cancer Specialists &amp; Research Institute at 1037 S. State Road 7, Suite 303, Wellington, FL 33414.
