YOU ARE NOTIFIED that an action for Change of Name for a Minor and/or Dependent Child has been filed against you and that you are required to serve a copy of your written defenses, if any, to it on Paul H. Bowen, Esquire, Attorney for Petitioner, whose address is Paul H. Bowen, P.A., P.O. Box 2067, Palm Harbor, Florida 34682, on or before 28 DAYS after the first publication of the above notice, and file the original with the Clerk of the Circuit Court at the Robert D. Sumner Judicial Center, 38053 Live Oak Avenue, Suite 207, Dade City, FL 33523-3805 before service on Petitioner or immediately thereafter. If you fail to do so, a default may be entered against you for the relief demanded in the petition.
Copies of all court documents in this case, including orders, are available at the Clerk of the Courts office. You may review these documents upon request.
You must keep the Clerk of the Circuit Courts office notified of your current address. (You may file Designation of Current Mailing and E-Mail Address, Florida Supreme Court Approved Family Law Form 12.915.)
