NOTICE IS HEREBY GIVEN that the undersigned, desiring to engage in business under the Fictitious Name of
Baptist Medical Laboratories located at 3563 Philips Highway, Building D, in the City of Jacksonville , Duval County, Florida 32207 intends to register the said name with the Division of Corporations of the Department of State, Tallahassee, Florida.
