Physician Partners of America announces that Lee Neubert, DO, will be departing the practice effective April 19, 2026. We want to reassure patients that this will be a smooth transition and they will continue to receive exceptional patient care with Dr. Alejandro Tapia moving forward. Physician Partners of America will retain all patient medical records. Patients who wish to obtain a copy of their records may do so by contacting the Medical Records Department at medicalrecords@physicianpartnersoa.com or by calling 800-400-PPOA.
