NOTICE IS HEREBY GIVEN, pursuant to  456.057, Fla. Stat. (2014) and Rule 64B8-10.002, F.A.C., that Dr. Philip Oranburg, M.D. is retiring from the practice of medicine with Philip Reid Oranburg, M.D. PA, effective as of March 27, 2026and that Yoel R. Vivas, M.D., P.A. d/b/a The Arrhythmia Center of South Florida has been designated as records owner; thus, any requests by patients for copies of medical records can be made directly to Yoel R. Vivas, M.D., P.A. d/b/a The Arrhythmia Center of South Florida at 1590 NW 10th Avenue, Suite 404, Boca Raton, FL 33486.
