Public Notice of Voluntary Termination of Medicare Provider Agreement Primary Medical Staffing, Inc., a home health agency with locations at 3600 Red D, Ste 303, Miramar, FL 33025 and 8000 Oak Ln, Ste 102, Miami Lakes, FL 33016, hereby provides public notice at least 15 days before the effective date of the termination of its Centers for Medicare and Medicaid Services contract and certification. The public is informed that as of May 1, 2026, Primary Medical Staffing, Inc. shall no longer maintain a contract or certification with the Centers for Medicare and Medicaid Services wherein it provides home health goods and services to individuals insured or certified for Medicare coverage. As such, Primary Medical Staffing, Inc. will no longer provide home health goods or services to individuals insured or certified for Medicare coverage from either of its locations following May 1, 2026.
