NOTICE TO CREDITORS The name of the Decedent, the designation of the court in which the administration of this estate is pending, and the file number are indicated above. The address of the court is 200 Ohio Avenue South, Suite 106, Live Oak Florida 32064. The names and addresses of the personal representative and the personal representative's attorney are indicated below. The personal representative has no duty to discover whether any property held at the time of the Decedent's death by the Decedent or the Decedent's surviving spouse is property to which the Florida Uniform Disposition of Community Property Rights at Death Act as described in ss. 732.216-732.228, Florida Statutes, applies, or may apply, unless a written demand is made by a creditor as specified under s. 732.2211, Florida Statutes.
