You are hereby notified that your eligibility to vote is in question. You are required by section 98.075(7) Florida Statutes, to contact the Supervisor of Elections in Palatka, Putnam County, Florida, no later than Thirty (30) days after the date of the publication. Failure to respond will result in a determination of ineligibility by the Supervisor and your name will be removed from the statewide voter registration system. Within the THIRTY (30) days you may contact the Supervisor of Elections at 386-329-0224 for further assistance or to receive information regarding the potential ineligibility and the procedure to resolve this matter. If you attempt to vote at an early voting site or your normal election day polling place, you will be required to vote a provisional ballot. If you vote by mail, your ballot will be treated as a provisional ballot. In either case, your ballot may not be counted until final determination of eligibility is made. If you wish for your ballot to be counted, you must contact the supervisor of elections office within 2 days after the election and present evidence that you are eligible to vote.
 
 
Por medio de la presente, se le avisa que su elegibilidad para votar est en duda. En virtud de la seccin 98.075(7) de los Estatutos de la Florida, se le exige que se comunique con el Supervisor Electoral en Palatka, Condado de Putnam, Florida, a ms tardar Treinta (30) das despus de la fecha de la publicacin.
