Notice of Data Security Incident Fort Lauderdale, Florida February 6, 2026 On February 6, 2026, Chapman & Associates PC notified certain individuals that their personal information may have been compromised as part of a security incident. Chapman & Associates PC takes the privacy and security of information in its possession very seriously and sincerely apologizes for any inconvenience this incident may cause. This notice is intended to alert potentially impacted individuals of the incident, steps we are taking in response, and resources available to assist and protect individuals. What Happened On or about January 31, 2025, Chapman & Associates PC detected suspicious activity within its network. Upon discovering the suspicious activity, Chapman & Associates PC immediately secured its environment and promptly engaged third-party forensic specialists to investigate the extent of the unauthorized activity. After a forensic investigation, Chapman & Associates PC determined an unauthorized third party accessed and acquired certain individuals personal information during this incident. Chapman & Associates PC have provided written notice to all impacted individuals. We have no reason to believe that any individuals information has been misused as a result of this event. What Information Was Involved Again, we found no evidence that any individuals information has been specifically misused. However, the following information was potentially exposed to an unauthorized third party: first name, last name, drivers license number, passport number, and medical information. Notably no Social Security numbers or financial account information were exposed for any individuals. What We Are Doing Data security is one of our highest priorities. Upon detecting this incident, we moved quickly to initiate a response, which included conducting an investigation with the assistance of IT specialists and confirming the security of our network environment. We have also reviewed and enhanced our technical safeguards to prevent a similar incident.
