PHYSICIAN RETIREMENT
NOTICE IS HEREBY GIVEN, pursuant to  456.057, Fla. Stat. (2014) and Rule 64B8-10.002, F.A.C., that John M. Rayhack, M.D.is retiring from the practice of medicine with The Wrist and Hand Center, PA d/b/a Wrist and Hand Center effective as of March 20, 2026 and will no longer be available to see patients after this date.  John M. Rayhack, M.D. has been designated as records owner; thus, any requests by patients for copies of medical records can be made directly to John M. Rayhack, MD Wrist and Hand Center. 4728 N Habana Avenue  Suite 204  Tampa, FL 33614. (telephone number 813-879-7478) .  After March 30th , 2026 all requests for records can be made via online request at the web site:   WWW.RAYHACK.COM. Copies of this notice have been filed with the Florida Board of Medicine.
