Please provide a W-9, cost, and proof prior to publication . It will be necessary for this office to receive verification that this advertisement appeared four consecutive weeks with a copy of the advertisement attached to the verification for proof of service. Payment will be made by a State of Florida Purchasing Card (Visa credit card) upon receipt of the affidavit of publication.
