You are hereby advised that a petition for adoption has been filed and commenced in this Court. Said petition seeks to terminate your rights and obligations in respect to the child, and of the child arising to her from the potential parental relationship, including the right to inheritance, and that the child be placed for purposes of adoption with petitioner.
