NOTICE OF REQUEST FOR CLAIMS Notice is given that a Statement of Commencement of Winding Up with respect to New Millennium Smiles, LLC, a Georgia limited liability company (“Company”), with its registered office at 318-A Williamson Street, Savannah, Georgia 31401 has been delivered to the Secretary of State of the State of Georgia for filing in accordance with the Georgia Limited Liability Company Act (“Georgia Act”). Pursuant to Georgia Act § 14-11-608, persons with claims against the company must send via registered and certified mail a written notice of claim to the Company, to Attn: Claims Manager, 24 Navigator Lane, Savannah, Georgia 31410. The claim shall set forth, with specificity, the date the claim was incurred, the amount of the claim, the basis for the claim, and any and all other information relevant or necessary in order for the Company to determine the validity of the claim. Except for claims that are contingent at the time of the filing of the Statement of Commencement of Winding Up, or that arise after the filing of the Statement of Commencement of Winding Up, a claim against the Company not otherwise barred will be barred unless a proceeding to enforce the claim is commenced within two (2) years after the publication of this notice.
