Request for Bids Pioneer Center for Human Services is seeking bids for a rehabilitation project at one of its group home facilities including designing, demol-ishing, acquiring materials and building or installing the following components of the project: 1. new entryway with ramp to make it accessible and ADA compliant 2. replace kitchen cabinets hardware, countertop, sink and faucet 3. remodel bathroom to make it more accessible - replace bathtub with walk-in shower stall, install higher toilet and fixtures 4. removal of carpet, tile and replace with waterproof vinyl flooring 5. new carpeting, handrail on stairs and other miscellaneous. The project is funded with federal funding under the McHenry County Community Development Block Grant Program. Proof of Workman's Compensation and liability insurance must be submitted with sealed bid. Contractors and sub-contractors are responsible for all permits and inspec-tions. Contractors and subcontractors must be fully insured and cannot be debarred, suspended or otherwise excluded from receiving federal funding or working on state or federally funded projects. Prevailing wage requirements apply as bids must be submitted using applicable Davis-Bacon and Illinois Prevailing Wage rates (whichever is higher). Contractors and subcontractors are required to complete certified payroll sheets (Form WH-347). Women Business Enterprise and Minority Business Enterprise (WBE/MBE) and Section 3 businesses are encouraged to apply. McHenry County CDBG funded project regulations also apply - please see the following website for more information – www.mchenry countyil.gov/cd Information about each project can be requested via e-mail (PCProjects@ pioneercenter.org) or by calling (815) 344-1230. Site visits will be arranged on an individual basis. All bids must include all five (as listed above) components of the overall project and should also provide a separate cost estimate for each of the five component projects.
