PUBLIC NOTICE Southwest Aesthetics and Body Sculpture, LLC, 217 Vertin Blvd, Shorewood, IL 60404 will be closing as of April 13, 2026 ("Effective Date"). We encourage you to begin the process of establishing care with a new provider to ensure continuity of care. We appreciate the trust you have placed in us and want to ensure that you continue to have access to your medical records. If you have an upcoming appointment scheduled before the closing date, we will be happy to assist you with your transition during your visit. If not, you may continue your care with Dr. Sheikh or choose any provider of your preference. After the closure, patient records will be transferred to Morgan Records Management, LLC, the official records custodian. Records will be securely maintained for at least 10 years from your last date of service, in compliance with applicable laws.
