Cizek whose address was 134 Widsor Park Dr., Apt D302, Carol Stream, IL 60188. Letters of Office were issued on November 7, 2025 to Karen M. Carr, 8425 Autumn Dr., Woodridge, IL 60517 as: Independent Executor whose attorney is Michael Overmann. The estate Will be administered without Court Supervision, unless under section 28-4 of the Probate Act of 1975 (755 ILCS 5/28-4) any interested person may terminate independent administration at any time by mailing or delivering a petition to terminate to the Circuit Court Clerk.
