PUBLIC NOTICE OF SIGNIFICANT AMENDMENT TO THE FIVE-YEAR AND ANNUAL PHA PLAN The North Chicago Housing Authority (NCHA) hereby provides notice of a proposed Significant Amendment to its Five-Year and Annual Public Housing Agency (PHA) Plan. NCHA proposes to amend its plans to dispose of its Annual Contributions Contract (ACC) units and trans- fer associated housing assistance to the Lake County Housing Authority (LCHA), subject to approval by the U.S. Department of Housing and Urban Development (HUD). This includes 471 Housing Choice Vouchers (HCV), 70 Veteran Affairs Supportive Housing (VASH), 5 Family Unification Program Vouchers (FUP) and the Family Self-Sufficiency Program (FSS) along with the corresponding Annual Budget Author- ity, Administrative Fees and supporting grant funds.
