Creditor Notice: NOTICE OF DISSOLUTION Please take notice that Salem Medical Center Foundation, Inc., a New Jersey nonprofit corporation, has dissolved in accordance with the New Jersey Nonprofit Corporation Act. All claims against the company must be presented in writing by and mailed to Salem Medical Center Foundation, Inc., 165 Bridgeton Pike, Mullica Hill, NJ, 08062, Attn: Chief Legal Officer. Each claim must be presented within six month from the date of publication of this notice and must include the claimant's full name and address and the identity, title and complete contact information (including phone number and e-mail address) of the person presenting the claim on behalf of the claimant; a brief description of the facts giving rise to such claim, together with, copies of documentation supporting such claim; and a good faith estimate of the amount of such claim alleged to be due to the claimant.
