LEGAL NOTICE



NOTICE OF FORMATION of Birch Root Integrative Care, NP in Psychiatry and NP in Family Health, PLLC (“PLLC”). Articles of organization were filed by the NYS Department of State March 10, 2026. PLLC’s office is in Warren County, NY. The NYS Secretary of State (“SOS”) is designated as PLLC’s agent upon whom process against PLLC may be served. The SOS shall mail any process served upon the SOS for PLLC to PLLC at 420 Quaker Road, Suite 1, Queensbury, NY 12804. Purpose: Nurse Practitioners in Psychiatry and in Family Health.
