FOSTER A Petition having been filed in this Court by the Westchester County Department of Social Services alleging that you neglected children for whom you were legally responsible as a parent. YOU ARE HEREBY SUMMONED to appear before this Court FOR AN APPEARANCE at 131 Warburton Ave, Yonkers, NY 10701 on the 29th Day of April at 09:30 AM of said day, to show cause why the Court should not enter an order of neglect and custody of the child to the Petitioner, Westchester County Department of Social Services, as provided by law. PLEASE TAKE NOTICE, that if your child enters foster care for 15 out of the most recent 22 months, the Department of Social Services may be required by law to file a petition to terminate your parental rights and my file before the end of the 15-month period. If guardianship and custody of the child is committed to the Westchester County Department of Social Services, the child may be adopted with the consent of the Westchester County Department of Social Services, without your consent or further notice to you. PLEASE TAKE FURTHER NOTICE, that you have the right to be represented by a lawyer, and if the Court finds you are unable to pay for a lawyer, you have the right to have a lawyer assigned by the Court.
