NOTICE OF FORMATION OF PROFESSIONAL SERVICE LIMITED LIABILITY COMPANY: The name of the professional service limited liability company is: Matthew T. Bennett, MD, PLLC (the "Company"). The date of filing of the Articles of Organization of the Company with the Secretary of State was March 31, 2026.
