PUBLIC NOTICE To: Joseph Gardner 929 N 3rd Street, Ironton, OH 45638 On March 17, 2025, the Ohio Chemical Dependency Professionals Board (OCDP) issued a Notice of Opportunity for Hearing case no.CO2025-5684 pursuant to O.R.C. 119 to Joseph Gardner (Mr. Gardner). The NOO stated OCDP intends to take action against Mr. Gardner's CDCA certificate (#190973) in accordance with R.C. 4758.30. The Notice alleges that Mr. Gardner admitted to the Board's Investigator that they violated OAC 4758-8-01(C)(10). Mr. Gardner also failed to report his conviction history on the CDCAPre and CDCA applications. This is a violation of Admin. Code 4758-8-01(C)(3)(a).
