PETITION FOR ADOPTION
To: Holly Ann-Lynn Cikalo, whose whereabouts are unknown and cannot with reasonable diligence, be ascertained. You are hereby notified that on June 23, 2025, a petition for the adoption of William Lee Clyde Cikalo, a minor, whose date of birth is January 10, 2018, was filed in Clark County Probate Court; and the consent and best interest hearing on said petition will be had before said Court on April 22, 2026 at 2:30 P.M. It is alleged in the Petition for Adoption, pursuant to R.C. 3107.07, that the consent of Holly Ann-Lynn Cikalo is not required due to the following:
(X) the parent has failed without justifiable cause to provide more than de minimis contact with the minor for a period of at least one year immediately preceding the filing of the adoption petition or placement of the minor in the home of the petitioner.
(X) the parent has failed without justifiable cause to provide for the maintenance and support of the minor as required by law or judicial decree for a period of at least one year immediately preceding the filing of the adoption petition or the placement of the minor in the home of the petitioner.
You have the right to be represented by an attorney of your choice. If you are unable to afford one, upon your timely request an attorney will be appointed at court expense. You may request court appointed counsel by calling the Court at 937-521-1845, no later than ten (10) days before the above stated date for the consent Hearing.
