SUPERIOR CARE PLUS, LLC d/b/a SUPPORTIVE HOME HEALTH CARE & PATRIOT OUTPATIENT, LLC PROVIDE NOTICE OF A DATA BREACH Cincinnati, Ohio – The Journal News – April 29, 2026. Superior Care Plus, LLC d/b/a Supportive Home Health Care and Patriot Outpatient, LLC (collectively, the “Com- pany”) are writing to inform you of a recent data security incident that occurred on November 17, 2025, that resulted in the unauthorized access to certain patient information. This notice is intended to provide details about the incident, steps we are taking in response, and resources available to help protect against the potential misuse of personal information. What Happened? On November 17, 2025, the Company detected suspicious activity involving an employee email account.
