No.26-5-00016-29 SUMMONS AND NOTICE OF TERMINATION OF PARENT/ CHILD RELATIONSHIP (Hearing Date is Special Set provided by Court Administration) TO: MITCHELL ARTHUR CURRIER The Petitioners have filed with the Clerk of the above Court a Petition requesting that the parent/child relationship between the mother of the above-named child be terminated. A copy of the Petition is attached hereto.
