STATE OF WASHINGTON DEPARTMENT OF HEALTH Olympia, Washington 98504 BEGINNING OF REVIEW NOTICE Applicant, Project Description, and Estimated Capital Cost: Shalom Hospice of Northwest Washington, LLC proposes to establish a Medicare and Medicaid certified hospice agency in Whatcom County. The proposed agency will be located at 8487 Full Moon Way in Maple Falls [98266]. The estimated cost for this project is $54,000. Public Hearing: No public hearing will be held unless requested. To request a hearing, the request must be in writing and include the name, address, and signature of the person making the request. The deadline for requesting a hearing is Tuesday, April 21, 2026. If a public hearing is scheduled, the hearing notice will be published in The Bellingham Herald at least 15 days before the date of hearing. Public Comment Period: If no public hearing is held, written comments must be received by the Certificate of Need Program no later than Monday, May 11, 2026.
