BEGINNING OF REVIEW NOTICE
Certificate of Need Applications
Applicants, Project Descriptions, Estimated Costs:
The following five applicants are proposing to provide Medicare and
Medicaid-certified hospice services to the residents of Pierce County.
• Odyssey HealthCare Operating B, LP dba Gentiva: providing services
from an office in Seattle, WA, with estimated capital costs of $107,466.
• Golden Angels Hospice and Palliative Care, LLC: providing services
from an office in University Place, WA, with estimated capital costs of $32,000.
• Legacy Care Hospice, LLC: providing services from an office in Olympia,
WA, with estimated capital costs of $14,299.
• Northwest Healthcare Alliance, Inc., dba Assured Home Health &
Hospice: providing services from an office in Puyallup, WA, with estimated
capital costs of $150,000.
• Vitas Hospice Services, LLC: providing services from an office in Puyallup,
WA, with estimated capital costs of $143,755.
Public Hearing: No public hearing will be held unless requested. Requests must be in writing and include the name, address, and signature of the person making the request. The deadline for requesting a public hearing is Monday, May 4, 2026. If a public hearing is scheduled, the hearing notice will be published in The News Tribune at least 15 days before the date of hearing.
