TAKE NOTICE that a pleading seeking relief against you has been filed in the above-entitled action. The nature of the relief being sought is as follows: personal and property damages as more fully described in Plaintiffs Complaint alleging Negligent Operation of Motor Vehicle &amp; Negligence Per Se.
