Public Notice Pursuant to the Non-coal Surface Mining Conservation and Reclamation Act and the Clean Streams Law, notice is hereby given that Kosturick Construction, LLC, 8565 State Highway 285, Conneaut Lake, PA 16313, has made application to the Pennsylvania Department of Environmental Protection (DEP) for a permit to authorization discharge under an Individual NPDES Permit in association with the active non-coal surface mining activities known as Mullet Mine (SMP Number 20080301). Mullet Mine is located in East Fallowfield Township, Crawford County, PA. The proposed permit area is 41.6 acres and is situated on the north side of Atlantic Road (S.R. 3006) approximately 1600 feet east of the intersection of Atlantic-Lake Road (S.R. 3013),directly northeast of Atlantic, PA on the land principally of Mullet. The Conneaut Lake, PA U.S. Geological Survey7.5-minute topographic map contains the area described. The receiving stream for this proposed permit areais an unnamed tributary of Crooked Creek (TSF)then to Crooked Creek (TSF). The proposed operation includes one NPDES discharge points, which is located as follows: Point SPA (Lat: 40° 30' 28- Long: 80° 19' 58)is located approximately 1,500 feet direct east of Atlantic-Lake Road within the Mullet Mine permit boundary. The discharge point entersUnnamed Tributary#1 to Crooked Creekat its headwatersapproximately 300 feet southeast of where the tributary flows through a 30 culvert under Atlantic-Lake Road and will discharge surface runoff from areas disturbed by mining operations. Written comments, objections, or a request for public hearing or informal conference may be submitted to the PA Department of Environmental Protection, Knox District Office, White Memorial Building, 310 Best Avenue, P.O.
