City of Lake Forest Park PUBLIC NOTICE Application for Coverage Under the Washington State Department of Ecology Construction Stormwater NPDES and State Waste Discharge General Permit The City of Lake Forest Park, c/o Katie Phillips, 17425 Ballinger Way NE, Lake Forest Park, WA 98155-5556, has applied for coverage under the Washington State Department of Ecology's Construction Stormwater National Pollutant Discharge Elimination System (NPDES) and State Waste Discharge General Permit. The proposed project, SR-104 & 40th Place NE Roundabout, is located at approximately 18403 Ballinger Way NE, Lake Forest Park, King County, Washington. The project will disturb approximately 1.8 acres of soil associated with highway or road, residential, and utility construction activities. The receiving waters for this project are McKinnon Creek and Lyon Creek. Any person wishing to comment on this application, or on the Department of Ecology's proposed action, may submit written comments no later than 30 days following the last date of publication of this notice.
