NOTICE OF ACTION Polk County BEFORE THE BOARD OF MASSAGE THERAPY IN RE: The license to practice as a Licensed Massage Therapist Shixin Zhang, L.M.T. 260 Lakeside Ranch Circle Winter Haven, FL 33881 CASE NO.: 2024-47154 LICENSE NO.: MA 84839 The Department of Health has filed an Administrative Complaint against your license to practice as a Licensed Massage Therapist, a copy of which may be obtained by contacting Patricia Nelson, Assistant General Counsel, Prosecution Services Unit, 4052 Bald Cypress Way, Bin #C65, Tallahassee, Florida 32399-3265, (850) 558-9910. If no contact has been made by you concerning the above by May 15, 2026, the matter of the Administrative Complaint will be presented at an ensuing meeting of the Board of Massage Therapy in an informal proceeding.
