I, HERNANDEZ TAYLOR, a Alabama National declare, state and verify before a notary public me, who being first duly sworn, under oath desposes and says that affiant resides in and maintains a place of abode in the City of OKLAHOMA CITY, County of OKLAHOMA, State of OKLAHOMA, which he recognizes and intends to maintain as his permanent home; affiant declares that he also maintains a residence at 6608 N WESTERN AVENUE OKLAHOMA CITY, OKLAHOMA, and that he formerly resided at 800 FORREST AVENUE (city) GADSDEN (state) ALABAMA, but that his abode in Oklahoma constitutes his predominant and principal home, and affiant intends to continue it permanently as such. Affiant further declares that HERNANDEZ TAYLOR or the HERNANDEZ MARTEZ FAMILY OF TAYLOR ESTATE is an actual bona fide and legal resident of the State of Oklahoma, and the filing of this affidavit is to be accepted by all persons or any court as proof of such legal residence and permanent domicile.
