NOTICE OF GENERAL SURGERY CENTER CLOSURE
Effective May 15, 2026, Florida Health Care Plans (FHCP) will no longer offer general surgery services through its staff physicians. Patient medical records will be retained in a confidential and secure manner and will be available to patients or their authorized representatives. Records can be requested and obtained by sending an Authorization for Release of Medical Records to FHCP, 2450 Mason Ave., Daytona Beach, FL 32114, ATTN: Medical Records Dept., or by emailing medrecroi@fhcp.com.
