TO: Jonathan Javon Sirmons, alleged biological father. You are hereby notified that a female child named JJS was born to Davina Reminta Cox, in Fort Pierce, Florida, on April 21, 2013, and you have been identified as a possible biological father of the child. A Petition for Adoption of this child has been filed in the Superior Court of Columbia County, Georgia. Be advised that you will lose all rights to the child and will not receive notice of, nor be entitled to object to his adoption unless within 30 days of your receipt of the letter referred to in O.C.G.A 19-8-12 (c)(1), or date of last publication you file:
1) A Petition to Legitimate the child in the Superior Court of Columbia County, Georgia pursuant to the provisions of the Official Code of Georgia Annotated 19-7-22;
2) Notice of the filing of your Petition to Legitimate with the Adoption Clerk, Superior Court of Columbia County, Georgia, 640 Ronald Reagan Drive, Evans, Georgia, 30809; AND
3) Notice of the filing of your Petition to Legitimate with the attorney for the Petitioner at William C. Davison, 629 Ronald Reagan Drive, Evans, Georgia, 30809.
