NOTICE OF INTENTION TO REGISTER FICTITIOUS NAME The undersigned does hereby certify that Beach Town Enterprises, Inc is conducting a retail mattress and bedding business at 4135 Dr Martin Luther King Jr Blvd Orange 3, Fort Myers, FL 33916 in Lee County under the fictitious name of Beach Town Sleep Center and that said firm is composed of the following persons whose names and places of residence are as follows: Beach Town Enterprises, Inc 69 Meadowlark Cir Ellenton, FL 34222 Ownership of Beach Town Sleep Center is as follows: Beach Town Enterprises, Inc It is my intention to apply to the Department of State, Division of Corporations, State of Florida to register the said name of Beach Town Sleep Center under the provision of Chapter 90-267 (Fictitious Name Act), Laws of Florida (Section 865.09, Florida Statutes), 1990.
