NOTICE OF PRACTICE CLOSURE The Fertility Center of Orlando announces that it will be ceasing operation effective May 20, 2026. In compliance with the Health Insurance Portability and Accountability Act (HIPAA) and Florida Statute 456.057 and 395.3025, all patient medical records will be maintained in a secure and confidential manner. Patients of the Fertility Center of Orlando may obtain copies of their medical records by submitting a written request to: Attn: Medical Records - 1912 Boothe Cir Ste 200 Longwood, Fl 32750 Or by SMS/Phone: 407- 907-6471 Fax: 407-345-9007. Or Email: RecordRequest@fcobaby.com Requests for medical records must be received by July 20, 2026. After that date, records will be stored securely in accordance with Florida law.Medical records will be retained for a minimum of five (5) years from the date of the last patient encounter, or longer if required by law. For ongoing and future medical care, patients are encouraged to establish care with another qualified healthcare provider.We appreciate the opportunity to have provided your medical and reproductive care.
