Mail your written request to Florida Department of Agriculture and Consumer Services, Division of Licensing, P.O. Box 5708, Tallahassee, FL 32314-5708.

How long do I have to request a hearing?

You have 21 days from the date of the last publication to mail a hearing request before you waive the right.

What happens if I don't request a hearing?

If you don't request a hearing within 21 days, the Department will proceed and may suspend your license and eligibility.
