TO: Dr. David Smith: Take notice that an amended pleading seeking relief against you has been filed on May 6, 2026 and the amendedsummons was served at your place of employment on May 9, 2026, in the above-entitled action. The nature of the relief being sought is as follows:
This is an action in tort, seeking recovery of damages proximately caused by an intentional tort
pursuant to N.C. General Statute §1A-1, Rules 1, 3 and 4.
