  YOU are hereby commanded to appear before the County Court at Law No. 4, of Dallas County, Texas and file a written answer at George Allen Courthouse, 600 Commerce Street, Dallas, Texas at or before 10:00 o'clock a.m. of the first Monday after the expiration of 42 days from the date of issuance hereof, being Monday, 16th day of March, 2026, a Judgment may be taken against you.
  Said Plaintiff's Original Petition was filed on in cause number CC-25-07666-D, Styled CLINT LIVELY, Plaintiff (s) vs PGW AUTO GLASS, LLC; OEP GLASS TOPCO, INC.; LYNDA KING, INDIVIDUALLY AND D/B/A CLEAR CHOICE GLASS & MIRROR; CLEAR CHOICE GLASS & MIRROR; JEFF KING, INDIVIDUALLY AND D/B/A CLEAR CHOICE GLASS & MIRROR; TAMMY WEIGANDT VELMA, A KNOWN HEIR OF THE ESTATE OF ALFRED VELMA, JR., DECEASED; STEPHANIE YORK, A KNOWN HEIR OF THE EST ATE OF ALFRED VELMA, JR., DECEASED; FREDDY VELMA, A KNOWN HEIR OF THE ESTATE OF ALFRED VELMA, JR., DECEASED; VERONICA BEGARNEY, A KNOWN HEIR OF THE ESTATE OF ALFRED VELMA, JR., DECEASED; THE UNKNOWN HEIRS OF ALFRED VELMA, JR., DECEASED; KING CLEAR CHOICE GLASS AND MIRROR, LLC; PGW AUTO GLASS, LLC f/k/a ALL AMERICAN GLASS DISTRIBUTORS, INC.; CARLEX GLASS AMERICA, LLC; HAL M. The nature of plaintiff's demand being as follows: DAMAGES (COLLISION) (ACCOUNT).
STATEMENT 
  The nature of said suit being substantially as  follows, to wit: Suit for damages arising from a motor vehicle accident occurring on December 20, 2023, in Andeson County, Texas. Celsey Renea Lively was traveling in a 2018 Nissan Armada, northbound on. U.S. Highway 287, near County Road 1435, in Anderson County, Texas. Alfred Velma, Jr., was operating a 2022 Toyota Tacoma pickup truck owned and/or leased by Defendants, PGW AUTO GLASS, LLC, OEP GLASS TOPCO, INC., PGW AUTO GLASS, LLC f/k/a ALL AMERICAN GLASS DISTRIBUTORS, INC. and/or CARLEX GLASS AMERICA, LLC and hauling a trailer owned and/or leased by Defendants, PGW AUTO GLASS, LLC, OEP GLASS TOPCO, INC., PGW AUTO GLASS, LLC f/k/a ALL AMERICAN GLASS DISTRIBUTORS, INC. and/or CARLEX GLASS AMERICA, LLC, traveling southbound on U.S. Highway 187, while in the course and scope of employment with Defendants, PGW AUTO GLASS, LLC, OEP GLASS TOPCO, INC., PGW AUTO GLASS, LLC f/k/a ALL AMERICAN GLASS DISTRIBUTORS, INC., and/or CARLEX GLASS AMERICA, LLC, near County Road 1435, and approaching Celsey Renea Lively's vehicle, in Anderson County, Texas. Alfred Velma, Jr. failed to control the speed of his vehicle, failed to control his vehicle, failed to drive in a single lane, and crossed over the center of the highway and into the northbound lanes of travel, and into Celsey Renea Lively's lane of travel, colliding with Celsey Renea Lively's vehicle, subjecting Celsey Renea Lively to tremendous force and fatally injuring and killing Celsey Renea Lively 
  If this citation is not served within ninety days after the date of its issuance, it shall be returned unserved. The officer executing this process shall promptly execute the same according to law, and make due return as the law directs.
