Box 2007 Pikeville KY 41501 is applying to the Virginia Division of Mined land Reclamation for a Renewal/Reissuance of PERMIT No. 1202411, in order to continue the operations at the following location. Guest Mountain No. 5 Mine is located 3.5 miles north of Stonega on Callahan Creek in Wise County, Virginia at Latitude 3551472.16 and Longitude 10227701.93. The permit area is 64.35 acres and is contained on the Appalachia U.S. Geological Survey 7.5 minute quadrangle map. The Renewal Map showing the permit area is attached in Section 21.2 of the application. Drainage and wastewater resulting from the permitted operation will be discharged into Callahan Creek, via sediment pond treating surface runoff and existing underground mine discharge. The discharge locations may be viewed on the Monitoring Point Location Map attached in Section 21.2 of the application. Any person with a valid legal interest which might be adversely affected by this proposal, or an officer or head of any federal, state or local government agency or authority, may within 30 days of May 29, 2026 submit written comments or objections concerning this application, and may also request, in writing, that the Mined Land Repurposing Program hold an Informal Conference concerning this application.
